13. How to Add a Behaviour Incident in TrackAble
The Behaviour Module in TrackAble allows staff to record and track incidents quickly and securely. This guide explains how to access the module, add a new incident, and understand each field in the form.
Accessing the Behaviour Module

From the left-hand menu of the Assessment screen, click Behaviour.

You will be taken to the Behaviour Module, where all existing incidents are listed.
How Incidents Are Displayed

Incidents appear in Date Added order (newest first).

You can re-order the list by clicking any column header.

Deleted incidents are automatically removed from view.

You will only see incidents allowed by your User Profile permissions.
Adding a New Behaviour Incident

Click Add Incident.

You will be taken to the incident form.

Any field marked with an asterisk (*) is mandatory.
Below is a breakdown of each field and how to use it.
Pupil Information
Pupil*

Click to view all pupils, or type the first few letters of their name to filter.

Select the correct pupil.
Important: Reports use the Pupil field to organise and filter incident data.
Other Pupils
If other pupils were involved:

Use the same search method to add them.
-
You can add multiple pupils.
Staff Information
Teachers Involved
-
Search and select teachers in the same way as pupils.

You may add more than one teacher if needed.
Site & Location
Site
-
For schools with multiple sites or multi-school hubs, choose the relevant site.
Location*

Once a site is selected, only locations belonging to that site will appear.

Select from the drop-down list.
Incident Overview
These fields are used for reporting and must be selected from the available lists.

Severity*
You can type to filter options.
If a required option is missing, please contact your TrackAble administrator.
Details and Actions
Details
Add a clear description of the incident.
Action
Record any actions taken in response to the incident.
Additional Options
Body Map
If the pupil was injured:

Click Body Map.
-
Select the area of injury on the front or back diagram.
Physical Intervention
Select from the drop-down list if physical intervention was used.
You can type to search for options.
If an option is missing, speak to your TrackAble administrator.
Behaviour Plan
Tick this if the pupil has a behaviour plan.
A text field will appear to provide more details.
Notifications
Default Administrator Emails
If your school has enabled behavioural alerts, all Behaviour Administrators will automatically receive a copy of the incident.
You can also add or manage additional staff here.
Email Summary
Enter email address(es) to send a copy of the incident immediately upon saving.

Separate multiple emails with a comma.

Useful for leadership notifications or pastoral follow-up.
Parents Informed
Document if and how parents were notified:

Who was contacted
-
Date and time
-
Method of communication (phone, email, meeting)
Safeguarding Flag
Safeguarding Incident
Tick this box if the incident is a safeguarding concern.
Only:
-
The person who created the incident
-
Users with safeguarding permissions
will be able to view it.
Saving the Incident
Once all required fields are completed:

Click Save.
-
The incident will appear in the Behaviour list based on your sort order.
